In December 2006, San Francisco District Attorney Kamala D. Harris reported that criminal charges had been filed against two defendants for perpetrating workers' compensation fraud.

According to a release from the San Francisco District Attorney's Office, Dennis Robert Fisher, a workers' compensation claim handler with American Commercial Claims Administrator (ACCA), partnered with Colleen Moore, a licensed practical nurse. Together, the duo hatched a scheme that involved Fisher making illegitimate payments to Moore for services and care she wasn't offering. In one instance, Moore was even hired to provide "care" to a deceased person. According to court documents, the duo's fraudulent activity occurred from July 2002 until September 2004, and resulted in fraudulent payments totaling more than $246,000.

Both Fisher and Moore have been charged with embezzlement, grand theft, falsifying corporate records. Bail was set at $250,000 for each defendant.
