An Elgin, Illinois man who submitted claims to several insurers in phony slip-and-fall incidents could serve between four and 15 years in prison after pleading guilty to two felonies, including one count of aggravated insurance fraud and one count of forgery.

The National Insurance Crime Bureau (NICB) was instrumental in prosecuting 52-year-old Juventino Guzman, who entered a guilty plea on Nov. 6. The organization initially tipped off local police after noticing that Guzman used his own name and "Prodigio Guerrero" when filing claims with Westfield Insurance, Hartford Insurance, and Iowa Mutual Insurance for the alleged slip-and-falls.

Guzman primarily targeted fast food chains.
